What to Bring
Family member or friend
An oncology consultation provides lot of information which can feel overwhelming. Cancer Center physicians and support staff are here to help however, a family member or friend can help listen and provide support.
Medical record / summary
A copy of your medical records detailing medical problems and any related treatment history. Please have your current doctor send a copy of medical records to the Cancer Center in advance of the appointment. These records include:
- Radiology Test Results - xrays, CT Scans, MRIs, Mammograms, Ultrasounds, PET scans, bone scans, etc.
- Pathology Slides - Biopsy results from the doctor who diagnosed cancer to obtain a Pathology Report
- List of Medications - including dosage information
Valid photo identification
Driver's license, state ID, passport, etc.
Current insurance card
Provide us with complete health insurance information and be prepared to pay your portion at the time of service, or make payment arrangements.
List of current physicians
Including addresses and phone numbers.
List of allergies
Notebook
Or a laptop/tablet or cell phone using the Cancer Center's free wireless network will be useful for writing notes and questions.

Advance Care Planning
Advance care planning is an ongoing process of planning and documenting future healthcare priorities and values in the event that medical decision-making is no longer possible. If a plan is not already in place, discussions should begin at the time of a diagnosis.
