Care Team Models Improve Patient Care

Sansum Clinic is now using a care team model approach to better meet the healthcare needs of our patients, to engage them further in their care and to improve their overall experience with us.  

A care team’s members work together to use their skills to the fullest for the benefit of each patient. Each care team includes: 
  • A primary care doctor 
  • Advanced practice clinicians (APCs) like a nurse practitioner or physician assistant 

Advanced practice clinicians are highly trained clinicians who blend clinical expertise in diagnosing and treating health conditions with an added emphasis on disease prevention and health management. They can order labs, imaging and additional studies, prescribe medicine and address all a patient’s healthcare needs and concerns.  

Patient Benefits of a Care Team Model: 

  • Increased Access – patients will have more appointment options and be better able to receive the right care at the right time. Same-day appointments are often available with APCs, reducing wait times. 
  • Comprehensive Care – a diverse care team with a broad range of expertise and knowledge combined with collaboration on complex cases leads to more well-rounded care of patients. 
  • Wider Range/Depth of Care – APCs often spend more time with patients, allowing for greater focus on preventive care, chronic disease management and patient education. 
  • Customized Care – care team models have more clinicians available to the patient, allowing for greater tailoring of treatment plans and care strategies designed for each patient's unique situation  
  • Better Outcomes – Care team models lead to better outcomes because chronic conditions are managed better, patients are less likely to be readmitted to the hospital. 
  • Increased Patient Satisfaction – Patients indicate they appreciate added support and attention which leads to higher care satisfaction rates.  
  • Cost Effectiveness – potential reduction in healthcare expenses for patients and healthcare organizations because APCs can provide the same services as physicians at a lower cost.
